Job Description:

The Medical Coordinator is responsible for scheduling medical and health appointments, as well as coordinating follow-up care. In collaboration with the Nursing Department, staff, and management, the Medical Coordinator must ensure that individuals receive all necessary treatments and care in accordance with state and federal regulations, agency policies and procedures, and the agency’s mission. The role of the Medical Coordinator is to supervise and coordinate health follow-ups, treatments, referrals, indications, or protocols prescribed by healthcare practitioners and the agency nursing department in the Residence (IRA or ICF).

Qualifications:

  • Associate’s degree in the Human Service-related field (or equivalent credits), with 2 years’ work experience in the OPWDD and serving the intellectually developmentally disabled population.

  • Or Medical Assistant/CNA with 3 years’ work experience in the OPWDD and serving the intellectually developmentally disabled population

  • Or High School Diploma/GED with 5 years’ work experience in the OPWDD and serving the intellectually developmentally disabled population.3+ years working experience in Behavioral Health or an intellectual disability-related field.

  • Meet and clear all regulatory standards.

  • Experience conducting medical appointments and scheduling appointments, administering medication, and correctly documenting medication dispensed.

  • New York State Driver’s license is required.

  • Preferably trained in SCIP-R, AMAP, and CPR/First Aid.

  • Knowledge of basic human physical and psychological needs.

  • Knowledge of simple health and safety precautions. Knowledge of simple nursing care, first-aid, behavior management, personal and environmental hygiene.

  • Ability to write progress and behavior reports using a professional, objective style and voice.

  • Ability to act as a liaison between program planning teams.

  • Ability to use computers and other electronic equipment.

  • Ability to promote self-advocacy for individuals during medical assessments.

Essential Job Duties and Responsibilities:

The following duties are essential to the role. Other duties may be assigned as necessary.

  • Responsible for monitoring and ensuring that daily documentation for individuals’ programming is complete and accurate.

  • Conduct Meds counts

  • Ensuring goals are met in areas including client and family satisfaction, safety, quality, and team member performance.

  • Conduct compliance reviews of the medical and program records to ensure state and agency records and program compliance, monitor and review all general events and incidents, and develop systems to ensure the health and safety of individuals.

  • Primary contact and communication regularly with doctors’ offices, medical staff regarding programming questions and health information.
